O’Neil spent his youth career playing for Portsmouth. He turned pro in 2000 when he was 16 years old. He stayed with the team through 2007. At that time, he was also loaned to Walsall and Cardiff City.

In 2007, he signed with Middlesbrough and with the team through to 2011. The signing contract was for a reported 5 Million Pounds.
After his time with Middlesbrough, he went on to play for West Ham United, Queens Park Rangers, Norwich City, Bristol City, and the Bolton Wanderers. In 2019, he announced his retirement and in 2022, he started managing AFC Bournemouth.
