At the top, although Liverpool have been crowned champions they received the trophy from former plater and manager Sir Kenny Dalglish at Anfield on Wednesday night - and Manchester City have been confirmed in second place, the identity of the two teams who will join them in the Champions League next season has yet to be determined.
It is currently between three sides Manchester United, Chelsea and Leicester, and, in an intriguing twist of fate, United must visit Leicester on the last day of the season.
United could have given themselves a cushion by beating West Ham on Wednesday night, but they could only manage a draw, meaning that they go into the final match one point and two places better off than Leicester, with an identical goal difference.
They now must avoid defeat on Sunday to ensure a top four finish.
Chelsea have the same number of points as United and must similarly avoid defeat in their game with Wolves, or match the Leicester result.
Meanwhile, at the other end of the table, Bournemouth, Watford, and Aston Villa are all vying to avoid joining already relegated Norwich City in the Championship next season.
Bournemouths plight is the worst of the three. They have three points fewer than their two main rivals and need to beat Everton away on Sunday, and hope that their two rivals lose.
Watford, who fired manager Nigel Pearson last weekend, now know that their fate is no longer in their own hands after they were heavily beaten at home by Manchester City on Tuesday. They travel to Arsenal and will be hoping the Gunners are in generous mood and are focusing on their FA Cup Final next week.
The Hertfordshire side need to achieve a better result than Aston Villa, who gave their own hopes of survival a massive boost, by beating Arsenal at home on Tuesday. Villa must visit West Ham, whose point at Old Trafford means they are mathematically safe for another year.
Whatever happens, Sunday promises to be a tense time for the fans of all the teams involved in these key matches, and there are likely to be a few twists and turns before the final league standings are confirmed.
