Chad Qualls
A relief pitcher for the Arizona Diamondbacks, Chad Michael Qualls is particularly known for the eerily similar grand slams he allowed to the Chicago White Sox. The first one was in the 2005 World Series, when Qualls pitched for the Houston Astros, he gave up a grand slam on the first pitch in the 7th inning to Paul Konerko, to give the Sox the lead, during Game 2.
The second grand slam was in 2006, when the Astos and Sox once again met up in an interleague rematch, for the previous year's World Series. Qualls served up a grand slam on the first pitch in the 7th inning to third baseman Joe Crede to tie the game. Furthermore, both grand slams were hit at U.S. Cellular Field, into the left field stands.
In 2007, Qualls was traded to the Arizona Diamondbacks, along with Juan Gutierrez and Chris Burke, in exchange for Jose Valverde. In mid-September 2008, Qualls replaced Brandon Lyon as closer, after Lyon was unable to maintain form in the second half of the season.
